 (1) Your Name
 (2) Name of your partner, if there is.
 (3) Your email address
 (4) Say yes if you need a car ride.  If yes, where do you live ?
 (5) Say yes if you can provide others a car ride.  If yes, how many is it ?
 Then, car allocation will be emailed to you at least three days prior to the
 event.   If you are not coming from the Tokyo area, we will pick you up at
 some train station near the hotel area.
